The Single Assessment Process (SAP) for Older People was introduced in the National Framework for Older People. The purpose of SAP is to ensure that older people receive appropriate, effective and timely responses to their health and social care needs, and that professional resources are used effectively. CareWorks has developed CareWorks RAISE SAP to support authorities in taking the transitional steps to electronic SAP, whilst maintaining the fundamental purpose of ensuring that older people receive appropriate, effective and timely responses to their health and social care needs, and that professional resources are used effectively. CareWorks RAISE SAP is designed to allow Local Authorities to create their own assessment forms providing users with a fully configurable and flexible assessment framework to complete, store, view and share those assessments. This enables users of the system and health partners to be able to support integrated multi-agency working around all client groups. The CareWorks RAISE security component provides a secure multi-agency framework for professionals to work within. In addition, the component is able to meet the requirement of Self Assessment.
